A team in the Washington State Sandy Koufax Baseball Association, and certify that I am not to receive any compensation for playing. I further agree to abide by the rules of the Association and the American Amateur Baseball Congress.

In consideration of acceptance of this contract, I hereby for myself, my heirs, executors and administrators, waive and release any and all rights and damages I may have against the Seattle Rainier’s Youth Baseball Organization, American Amateur Baseball Congress and all member associations for any and all injuries suffered by me in games and practices for the team with which this contract is signed. I will return the jersey (team owned) and any other (team owned) at the end of the season or if I choose to leave the team before the end of the season for any reason, or if I choose to break the terms of this agreement. If the player is a minor, a parent or guardian MUST sign with player.

Contract valid for the 2010 baseball season only.
